Our Goal: To allow more high-risk families in New Jersey  to experience the benefits of the HealthySteps model, which integrates pediatric primary and behavioral care.

Project Description: This project aims to pilot the HealthySteps model at three pediatric practices at Hackensack Meridian Health.  Four staff will be hired by Hackensack Meridian Health and formally trained by the HealthySteps National Office on their clinical model.  Both organizations will work together to collect and analyze data during project implementation to help make the medical and business case for expansion of the HealthySteps model to high-risk families throughout New Jersey.
