Family Health Services is a component of Jefferson Health (formerly Kennedy Health), an integrated health system in southern New Jersey that offers a full array of healthcare services, both in the hospital and outpatient setting. A multi-site healthcare provider, Jefferson Health serves the residents of Camden, Burlington, and Gloucester counties. Family Health Services is an ambulatory outpatient facility that provides primary care services for low-income patient of all ages. It has two locations, in Somerdale and Washington Township.  

This project builds on a multi-year project that has supported the implementation of Cherokee Health System’s model of integrated care into Family Health Services at its Somerdale location. The current project aims to ensure the long-term financial sustainability of the model by assisting Family Health Services to improve its billing and coding practices. In addition, this project supports the expansion of the model to Family Health Services’ second clinic in Washington Township.
