The Visiting Nurse Association (VNA) Health Group is a nonprofit organization dedicated to helping individuals and families achieve optimal health and well-being by providing compassionate, coordinated, and innovative care in their homes and communities. As a safety-net provider, VNA provides home health care, hospice services, and community-based programs and services for those in need from birth to end of life.  Approximately 2,000 employees serve 126,000 patients and clients annually through 1,000,000 in-home and community-based units of service. 

This planning project is helping VNA determine the administrative, technical, and legal feasibility of building an integrated data system for its Child and Family Institute. This Institute comprises 16 separate programs, including four Community Health Centers that serve more than 10,000 patients. Each of these programs use different data systems, which makes the coordination of care and services difficult. If the system is successfully implemented, it will not only help improve the quality of VNA services but also serve as a model for other agencies and communities.
